Health insurance can be very complex and difficult to understand if you are not experienced with it. Insurance

I am in-network some insurance. When I am in-network, I will bill your insurance for their portion of the bill. Please pay your copayment at the time of each appointment with Venmo or Zelle.  If you prefer to keep a credit card on file for me to charge, that is fine, too.

  • Blue Cross/Blue Shield PPO and Blue Choice PPO: I am a preferred provider for Blue Cross/Blue Shield PPO. If you have a policy from Illinois Blue Cross Blue Shield PPO, I will be happy to verify your benefits and determine what your costs for treatment will be. You would only be responsible for the deductible or copayment; I will bill your insurance carrier for their share of your bill. I will contact BCBS to find out this information for you and we will discuss it.
  • Blue Cross/Blue Shield HMO: I am not an HMO provider. However, in recent years, when vigilantly pressed by the member, Blue Cross Blue Shield has reimbursed some of the session fee for HMO clients who see a BCBS PPO preferred provider like me. If you have BCBS HMO, you would pay me the full fee and I will send you a receipt that you can use to seek reimbursement. Call the member services number on your insurance card for further information.
  • Medicare: I am not a Medicare provider. If you have Medicare, and you want to see me, you will need to pay out-of-pocket and cannot get reimbursement from Medicare.
  • Other PPOs: Other PPOs may cover the majority of the cost of your appointments, if you have met the out-of-network deductible. I would be considered an “out-of-network provider.” You can call your PPO and check on the benefits. For more information on what to ask, click here. When I am out-of-network, I ask that you pay out-of-pocket and I will send you receipt that you can use to seek reimbursement from your insurance company.
  • Other HMOs: I do not contract with any HMO plans, so I would be considered an “out-of-network provider.” If you have HMO plan that is not Blue Cross Blue Shield, it is extremely unlikely that you would be eligible for any reimbursement for my services.

Other Payment Options

  • Flex Plan or Medical Savings Plan: If your employer has a tax-free medical savings plan, it is likely that this plan can reimburse you for any health care bills that you pay out-of-pocket (up to the amount that you have put in the plan for the fiscal year). Check with your employer for details.
  • Out-of-Pocket Payment: My out-of-pocket fee is $250 for the first two initial assessment meetings.  After that, the fee is $175 for a 45 minute session and $195 for a 55 minute session.  Scoring and interpretation of psychological testing is $175 per hour. I ask that a credit card by on file, but you can pay by Venmo if you prefer.

Good Faith Estimate

  • Health care providers are now required to provide a good faith estimate of charges to patients who are out-of-network.  
  • If you are coming to me for an ADHD assessment, and you are out of network, here is your good faith estimate: 2 hours of initial assessment meetings ($250 each) and one hour of psychological testing ($175) for a total of $675.  If you need a written psychological testing report, rather than simply a diagnosis for yourself and your treatment providers, let me know.  Additional hours of psychological testing, interpretation and write-up will be necessary.
